The role of oxidative stress on necrotizing enterocolitis in very low birth weight infants

La review riporta informazioni circa le ultime novità riguardo epidemiologia e patogenesi dell’enterocolite necrotizzante (NEC) e ruolo dello stress ossidativo nella precoce identificazione del neonato ad alto rischio.

La NEC è patologia devastante, peculiare dei nati pretermine, con un alto rischio di mortalità. L’etiologia è multifattoriale: eventi ischemici, infezioni, radicali liberi possono contribuire ad alterare la barriera gastriointestinale immatura. In particolare eventi critici, quali ipossia, iperossia, ischemia, ischemia-riperfusione, infezione/infiammazione, sono meccanismi che portano ad eccessiva generazione di radicali liberi, non contrastati da adeguate difese antiossidanti. Di conseguenza, a livello tissutale e cellulare, l’equilibrio redox si sposta verso la condizione di stress ossidativo.

Attualmente la diagnosi precoce di NEC è molto complessa. Indici clinici precoci non sono specifici e i dati di laboratorio non sono del tutto attendibili. Pertanto l’esordio tardivo della malattia, la sua rapida ingravescenza, la grave disabilità che ne consegue cosi come l’alto rischio di mortalità  richiede l’identificazione di biomarker precoci e specifici per NEC. C’è evidenza scientifica che i biomarker di stress ossidativo possano consentire la precoce identificazione dei neonati ad alto rischio di sviluppare NEC e che quindi possoao costituire le basi per sviluppare strategie terapeutiche nei confronti di una malattia tanto invalidante per il neonato pretermine.

Resuscitating preterm infants with 100% oxygen is associated with higher oxidative stress than room air.

The starting fraction of inspired oxygen for preterm resuscitation is a matter of debate and the use of room air in full-term asphyxiated infants reduces oxidative stress. This study compared oxidative stress in preterm infants randomised for resuscitation with either 100% oxygen or room air titrated to internationally recommended levels of preductal oxygen saturations.

METHODS: Blood was collected at birth, two and 12-hours-of-age from 119 infants <32 weeks of gestation randomised to resuscitation with either 100% oxygen (n = 60) or room air (n = 59). Oxidative stress markers, including advanced oxidative protein products (AOPP) and isoprostanes, were measured with high performance liquid chromatography and mass spectrometry.

RESULTS: Significantly higher levels of AOPP were found at 12 hours in the 100% oxygen group (p<0.05). Increases between two and 12-hour AOPP (p=0.004) and isoprostanes (p=0.032) concentrations were significantly higher in the 100% oxygen group.

CONCLUSION: Initial resuscitation with room air versus 100% oxygen was associated with lower protein oxidation at 12-hour and a lower magnitude of increase in AOPP and isoprostanes levels between two and 12-hours-of-life. Correlations with clinical outcomes will be vital to optimise the use of oxygen in preterm resuscitation

 

Proton nuclear magnetic resonance spectroscopy of urine samples in preterm asphyctic newborn: a metabolomic approach.

In order to highlight differences in the metabolic profile of healthy (control) compared with asphyxiated newborns, by using untargeted metabolomic approach coupled with (1)H NMR spectroscopy, we evaluated the effects of asphyxia on newborn urine metabolites. Our results showed that lactate, glucose and TMAO, together with threonine plus 3-hydroxyisovalerate are the metabolites more characterizing the asphyxiated group; lower contribute to discrimination is related to other metabolites such as dimethylglycine, dimethylamine, creatine, succinate, formate, urea and aconitate. After 24-48h from resuscitation preterm asphyctic neonates showed their recovery pattern that still can be differentiated by the controls

Progetto sottoposto a competizione per finanziamento in seno ai Progetti di Ricerca Scientifica di Alta Rilevanza Nazionale

Intrauterine growth restriction (IUGR) is a major complication of pregnancy that results in marked fetal, neonatal and long-term morbidity and mortality.

Reduced fetal growth is seen in about 10% of pregnancies. The spectrum of IUGR phenotype is pleiotropic and actual understanding of the multidimensional underpinnings of IUGR pregnancy lacks to provide an adequate picture of the overall condition of the affected fetuses. As a consequence, the decision to deliver a preterm IUGR fetus still remains one of the great challenges in medicine. Feto-placental hypoxaemia, oxidative stress (OS), inflammation, umbilical cord morphology and cell components including stem cells, have been described to be involved in IUGR pathogenesis.

This project aims to go in depth on pathogenetic mechanisms of IUGR allowing to setting up an integrated biomarkers panel to classify IUGR pregnancies into different sub-diagnostic categories and carefully identify fetuses who effectively would benefit of early intervention.

To this end we will perform a collaborative project combining the different skills of university laboratories with a well-recognized experience in the field. The expertise of each participant will be unified as in a jigsaw puzzle in which all dowels point out and unravel mechanisms of the heterogeneous pathogenesis of IUGR pregnancy.

Omics techniques (metabolomics, miRNA) will be used to investigate complex fetal response to an adverse intrauterine environment.

mRNA, OS biomarkers, and metabolomics will be analyzed in mothers and in cord blood, miRNAs, autophagy and ER stress, histological and immunohistochemical studies will be performed in umbilical cord.

This project has the potentiality to provide a better clarification of the role of intrauterine factors in pregnancy allowing to identify those babies diagnosed with IUGR that are more likely to have complications during pregnancy, during delivery and afterward, grading the risk on the knowledge of what caused the growth problem and how severe the growth restriction is. The identification and validation of new indices of complexity to be used in the clinical routine will reduce concerns and uncertainity about how and when to intervene  in case of IUGR diagnosis.

In conclusion, the better knowledge of IUGR pathology together with a new integrated panel of biomarkers profiles should allow to improve decision-making, to implement new treatment strategies, addressing the critical needs of pregnant women and their offspring. Ad hoc startup among  the universities involved in the project will take care of all the technological  potentialities including to develop one or more integrated diagnostic kits. It is reasonable that, if this project will contribute to reduce at least of 10% the incidence of low birth weight and low gestational age, thought better, personalized  management of IUGR pregnancies, it will reach the goal to effectively  reduce the financial and social enormous burden of IUGR pregnancies.

Lipid and protein oxidation in newborn infants after lutein administration.

Obiettivo dello studio: verificare l’ipotesi che la supplementazione con luteina neonatale nelle prime ore di vita riduce lo stress ossidativo neonatale (OS ) nell’immediato periodo post-partum .

Metodi: è stato condotto un trial clinico randomizzato arruolando 150 neonati a termine divisi in due gruppi: trattati ( n = 103, 0.28 mg di luteina in gocce, per via orale) e controlli ( n = 47, 0.5 ml di soluzione glucosata al 5%) . I campioni di sangue sono stati prelevati alla nascita dal cordone e a 48 ore dopo il parto , in occasione dell’effettuazione degli screenings neonatali. In tali campioni sono stati valutati: idroperossidi totali ( TH ) , prodotti avanzati di ossidazione delle proteine ​​( AOPP ) , e potenziale antiossidante totale ( BAP ). I dati ottenuti sono stati analizzati mediante test di Wilcoxon e analisi di regressione logistica multivariata .

Risultati . Prima della supplementazione di luteina, le concentrazioni ematiche medie di AOPP , TH , e BAP erano 36.10 umol / L , 156.75 mmol/H2O2 , e 2.361,04 umol / L nel gruppo dei neonati ‘trattati’. Dopo la supplementazione con luteina è stato riscontrato un incremento del BAP significativamente più alto e un più basso incremento dei TH nel gruppo dei ‘trattati’ rispetto ai ‘controlli’.

Conclusione . La supplementazione con luteina nei neonati a termine aumenta il loro potere antiossidante totale e riduce l’aumento degli idroperossidi totali conseguenti al “challenge iperossico” della transizione feto-neonatale. Lo studio è registrato al ClinicalTrials.gov NCT02068807 .

Il gruppo di Studio in Biochimica Clinica si propone di promuovere, sostenere e valorizzare la ricerca scientifica di interesse  neonatale e pediatrico e la sua applicazione alla protezione della salute in età evolutiva, pre e postnatale.

L’innovazione  nella ricerca scientifica si avvale della preparazione di progetti  di ricerca nazionale ed internazionali su tematiche riguardo:

1) identificazione precoce del neonato ad alto rischio di danno cerebrale (attraverso l’identificazione e la validazione di un profilo di markers biochimici specifici di danno cerebrale e il loro valore predittivo a breve e lungo termine)

2) tossicità dell’ossigeno

2) ruolo dei radicali liberi nelle malattie peculiari del neonato pretermine

3) approfondimento, anche mediante trials clinici, circa l’utilizzo e l’efficacia di vitamine e sostanze antiossidanti nel periodo perinatale quali melatonina e luteina
